Hotel Credit Card Request Form - If your teams needs a credit card for payment or a deposit on hotel or rental vehicle reservation, please complete this form and turn into treasurer.

Power chair agreement form - The form is to be completed by anyone borrowing a power chair from PS of Indy.

Reinbursement Form - If you have paid "out of pocket" for reimbursable expenses, please download this form and complete for reimbursement.

Staff Registration Form * - All staff, coaches, managers, referees and equipment managers who are unable to register online, please complete and mail to Power Soccer of Indy.
